Here is your form if you have a business you'd like to nominate, but aren't sure exactly WHAT category they fall into. Maybe you aren't sure if they have 10 or 20 employees. Nominate them below and tell us why, and we'll put them in the correct category for you.

Rules:

• No nominee may be awarded in consecutive years for any category. (See last year's winners HERE.)
• To be eligible for any award, the nominee must be a Member in good standing with the Humboldt Chamber of Commerce.
